Record the question, initial identifiers and where they came from. Include the review date. This helps distinguish prior knowledge from information first encountered in the report.
What was found and checked
For each important association, note its source, time context and independent verification outcome. Do not copy unnecessary personal details just to expand the note. Keep the context needed for your task.
What remains unknown
List contradictions and missing details separately. The conclusion should reflect the level of support. A later source can then resolve a specific disputed point without restarting the entire review.
